We are proud to offer our weight control
programs with a variety of choices and fee structures. After your initial new
patient introduction, consisting of the two steps outlined in the Program Information
section, and your initial follow-up visit with Dr. Wells, you are given a choice in
treatment options.
The fees for our program are as follows:
Initial Start Up

Appointment (will be scheduled within a week of the first appointment):

are give a choice of appointments:

We do not charge for weigh-ins or journal
reviews, but appointments are required.
We are not participating providers for any
insurance program. You may wish to call your insurance company to
inquire if they will cover our program, even if it is at out of network
pricing. We do not file medical insurance claims. We will, however,
be happy to assist you with questions on your insurance claims. Most insurance plans
do not cover weight control programs. Your insurance company may consider the
payment of your claim if there are other health concerns .If that is the case with
your treatment, we will discuss insurance filing with you after your initial visit.
A referral letter form your primary care physician might be helpful,
as Dr. Wells is the only Bariatric Specialist in the area.
**We would like to remind you that we require a
24-hour notice for appointment cancellations or rescheduling in order for you to avoid
being charged for a cancelled appointment and to allow us to make that appointment
available for someone else.
